CNA (licensed by the State of Texas) Certified Nurse Aid works closely with nurses and other clinicians to provide direct care to patients in their place of residence. A CNA may take blood pressure and pulse, temperature and respiration rates. CNA provides assistance to patients with transfers, ambulation, personal care such as bathing, feeding and dressing. CNA visits are based upon the plan of care as developed. Must complete patient visit notes in a timely manner Licensure must be current; CPR must be current. RN does field visits to the patient's residence. The RN must insure that the plan of care is being carried out as written and provides supervision to CNAs as needed. RN must be prepared to provide care, education to patient and family and needed and be source of support and/or refer patients to other clinicians as necessary. Must complete patient charts in EMR in timely fashion and be compliant with regulations and procedures. Recommend potential course of treatment and/or medication changes at IDT meetings. May perform admissions and make visits to pronounce. May be on rotating call Licensure must be current; CPR must be current. Must have own vehicle and proof of insurance. A homemaker provides routine task completion to insure the well being of the patient. This may mean helping with some daily living tasks such as meal planning and preparation, light housekeeping (doing dishes, vacuuming, dusting, cleaning bathroom), personal laundry, shopping and running errands. This is a per visit situation and not all activities or tasks are expected to be completed with every visit. A homemaker is not a patient sitter and hospice cannot provide that service.
